By signing this adoption contract, I certify that the information above is correct and completed and that I am at least 18 years of age. I realize any false information may result in losing the privilege of adopting an animal from Lake Villa’s Lost Angels. I understand that Lake Villa’s Lost Angels reserves the right to deny any requests for adoption. I authorize Lake Villa’s Lost Angels to verify any information and previous pet medical history. I understand that this application is the property of Lake Villa’s Lost Angels and may be shared with other adoption services. I hereby understand that by signing this legal contract that I , the adopter agree that I will in no shape or form alter the appearance of the animal unless deemed a medical reason by your veterinarian. This includes and isn’t limited to ear cropping, tail docking and declawing. If at anytime it is found that I have altered my pet, I may be ineligible from adopting in the future.
