Foster Agreement Form | Lake Villa's Lost Angels

Please read and check off each box afterward. *

I AGREE to care and love ANY foster pet given to me by LVLA as if it were my own.
I AGREE to provide the pet with exercise, basic obedience training and house training.
I UNDERSTAND that all pets placed in foster care need to be spayed/neutered and vaccinated. If I receive an unfixed pet, LVLA may request that I schedule an appointment with a designated LVLA veterinarian and assist in transporting the pet to and from the appointment. If I cannot arrange to do the transport myself, I will call my LVLA representative and assist in any way I can.
I UNDERSTAND that no pet CAN be adopted before being spayed or neutered. EXPENSES COVERED BY LVLA
I UNDERSTAND that all basic medical expenses for each foster pet that comes into my care will be covered by LVLA, provided that an authorized LVLA representative has approved the medical expenses, and the care is from a LVLA designated veterinarian.
I UNDERSTAND that if I choose to go to a vet of my choosing, I may be responsible for most of the vet bill.
I UNDERSTAND that LVLA will provide a carrier or collar/leash to any pet that comes into my care. LVLA appreciates willingness to donate toward the pet’s general expenses, like food, treats, etc., and LVLA will offer donated items, as available, for use by foster homes (Such general expenses, if paid for by a foster home and not by LVLA, may be tax deductible.) Crates can be lent to foster homes and returned to LVLA when a foster pet is adopted.

Pet Description and Photo on Website. *

I AGREE to assist in getting a photo of each foster pet that comes into my care on the LVLA website. I agree to either take a photo and write a short description myself, and submit it to my LVLA representative for review and assistance, or ask for assistance in taking the photo and writing the description of each foster pet in my care.
I UNDERSTAND that I can and should also advertise on Facebook, in local shops, on other websites, with friends, at the pet park, and anywhere else pets can be advertised for adoption.

Adoption / Transfer *

I UNDERSTAND that any potential adopter MUST be screened, interviewed and approved by an authorized LVLA representative. I understand that my input will be appreciated and a part of the process, and that the final decision to place or not to place any particular pet with any particular potential adopter will be made by an authorized LVLA representative.
I AGREE to let LVLA know if prospective adopters may contact me directly by phone or email, and to send copies of any email inquiries to my LVLA representative. If it is possible for a prospective adopter to visit my home to meet the animal, I will arrange for a LVLA representative to be there during the visit.

I understand that the following protocol should be followed regarding children in the house *

Do not under any under any circumstances leave a child unattended with any pet.
Do not leave bones around the house when children are present and be cautious and observant when children are eating and the pet is in the room.
If there is a child in the household under the age of 5 the pet must be in a crate or separate room while the pet is eating or sleeping to prevent accidents.
Children under the age of 13 will not walk the pet unless supervised by an adult.
Whenever a child is in the house with a pet, the child is at risk of being accidentally knocked over, scratched or bitten etc.
Should you have ANY concerns or feel uncomfortable with a situation between the pet and a child, contact your foster liaison or a senior LAKE VILLA’S LOST ANGELS Representative. We will either take the pet out of your home immediately or have a trainer come and meet with you, depending on the severity of the situation

Cats and Pets Co-Existing *

I understand that even if this animal has been housed with cats in its foster home that LAKE VILLA’S LOST ANGELS cannot guarantee the behavior with a new cat(s) will be the same. Please use caution when introducing a new pet to existing cats in the household and do the introduction slowly over a period of a few weeks, keeping the pet on leash. If you need assistance, please do not hesitate to contact a LAKE VILLA’S LOST ANGELS representative.

I understand that the following protocol should be followed when bringing additional pets into the house *

Do not leave multiple pets at home in the same room unattended.
Do not feed the pets in the same room and don’t leave toys and treats laying around the house.
LAKE VILLA’S LOST ANGELS can put you in touch with either a senior volunteer to assist you with a pet to pet introduction if necessary.
